METHODS: Fortyeight guinea pigs were divided into 4 groups with 12 each: gentamicin treated group (GM), Lysisin aspirin treated group (LAP), GM+LAP group and control group. Auditory brainstem response and cochlear preparation transmission electron microscope were used to evaluate the effects of hair cells on hearing threshold and the morphology of hair cells. Serum levels of GM were also tested. RESULTS: GM group developed a progressive threshold shift, reaching 50 dB at 8 kHz and the threshold shift in GM+LAP group was 30 dB (P<0.05). Injury in high frequency was significantly more severe than that in low frequency (P<0.01). Morphological results were consistent with the functional results. LAP did not affect serum levels of GM. CONCLUSION: The results suggest that LAP may be a promising therapeutic agent in reducing gentamicin ototoxicity. 【Keywords】 gentamicins/toxicity; lysisin aspirin; drug interactions 【摘要】 目的临床医学检验论文: 观察来比林对抗庆大霉素耳毒性的临床医学检验论文作用. 方法: 将豚鼠随机分为庆大霉素(gentamicin, GM)组、来比林组 (lysisin aspirin, LAP)、GM+LAP组及对照组,采用听性脑干反应(ABR)、耳蜗铺片及透射电镜技术、观察用药前后听阈及耳蜗毛细胞形态学改变,并检测血清尿素氮、肌苷以及庆大霉素血药浓度. 结果: GM组8 kHz ABR 4 wk平均阈移达50 dB,GM+LAP组平均阈移为30 dB,差异显著(P<0.05). 形态学改变与听力变化一致. GM+LAP组血液中丙二醛较GM组明显减少(P<0.05). GM+LAP组超氧化物歧化酶活性明显高于GM组(P<0.05). LAP对庆大霉素血药浓度没有影响. 结论: LAP能有效减轻GM的耳毒性作用,水杨酸盐连接有其他基团并不影响拮抗耳毒性的作用. 并且更加安全,有效,方便临床使用. 【关键词】 庆大霉素/毒性;来比林;药物相互作用 0引言 庆大霉素(GM)早期在临床上应用较为普遍 ,其耳毒性所致的感音神经性聋较为常见. 近年有研究报道部分自由基清除剂和铁螯合剂可以减轻氨基糖甙类抗生素的耳毒性[1,2]. 已有实验证实水杨酸钠作为一种自由基清除剂对庆大霉素耳毒性有预防作用[3,4]. 药物来比林是乙酰水杨酸与赖氨酸的复盐,水溶性好,酸度适宜,可作注射用,避免对胃肠道的刺激,并具有更为强力的解热镇痛作用. 我们旨在利用听性脑干反应(ABR)、耳蜗铺片及透射电镜技术观察用LAP前后听阈及耳蜗毛细胞形态变化,测定血清中丙二醛(MDA)超氧化物歧化酶(SOD)含量,测定肾功和血清GM浓度,观察来比林(LAP)预防庆大霉素耳毒性的作用.
