| | 医学论文集【摘要】 目的医学论文集 观察奥曲肽对急性坏死性胰腺炎(acute necrotizc pancreatitis,ANP)大鼠肺损伤的医学论文集治疗作用,并探讨其机制。方法 选用大鼠63只,在胰胆管内注射5%牛磺胆酸钠溶液诱导大鼠ANP模型。观察奥曲肽对ANP大鼠肺组织形态及细胞因子变化的影响。结果 奥曲肽治疗组较ANP+NS组肺湿/干重显著降低,PaO2明显提高,肺泡灌洗液中蛋白质含量[奥曲肽组为(339.9±72.5)mg/L]和白细胞计数明显下降或呈下降趋势。同时,血浆内毒素、血清TNFα和NO2-/NO3-水平及肺组织NOS活性显著降低,肺组织血管内皮细胞和导管上皮细胞一氧化氮合酶(NOS)表达下调,肺组织病理学改变减轻。结论 奥曲肽对ANP大鼠并发肺损伤有一定的治疗作用,其机制与其对内毒素、肿瘤坏死因子-α和一氧化氮等炎症介质的调节作用有关。 Beneficial effects of octreotide on the lung injury following experimental acute necrotic pancreatitis in rats 【Abstract】 Objective To investigate the possible beneficial effects of octreotide on the lung injury following acute necrotizing pancreatitis (ANP) and the mechanisms.Methods The ANP model of rat was established by injection of 5 % sodium taurocholate into the choledochus.Results Octreotide could decrease Wet/dry lung ratio significantly at all indicated time-point. At 12th h after induction of ANP, PaO2, protein content in BALF, plasma endotoxin level, serum NO2-/NO2-, and NOS activity were much lower in octreotide-treated rats than those in untreated group (P<0.05). Leukocyte count in BALF was also decreased with some exte nt but without statistical significant (P>0.05). Serum concentration of TNFα was lower dramatically in octreotide-treated group than in ANP+NS group. Pathological studies showed that inflammatory cell infiltration and pulmonary interstitial edema were improved, and type II epithelial cells were well preserved in octreotide-treated group.Conclusion Octreotide showed its beneficial effects on lung injury in both functional and morphologic alterations during the course of ANP. Regulatory roles of octreotide on pro-inflammatory factors, such as endotoxin, TNFα, and NO could be considered as one of the major mechanisms under this situation. 【Key words】 Acute pancreatitis; Somatostatin; Acute respiratory distress syndrome; Cytokine 我们以牛磺胆酸钠诱导大鼠,观察奥曲肽对急性坏死性胰腺炎(acute necrotic pancreatitis,ANP)大鼠肺脏形态和功能变化的影响以及对炎症介质的调节作用,现报道如下。 材料和方法 1.实验动物与主要试剂:雄性Spraque-Dawley大鼠63只,体重300~350 g。TNFα测试盒为Endogen公司产品。 2.方法:(1)ANP模型制备:大鼠实验前禁食12 h,自由饮水。ANP模型制备参照文献[1]。假手术组(sham operation,SO)大鼠仅作剖腹术。(2)实验分组:大鼠随机分3组:ANP+NS组(n=21)、ANP+奥曲肽组(n=21)和SO组(n=21)。ANP奥曲肽治疗组术后2、5、8、11 h分次于后肢根部皮下注射奥曲肽(Novartis公司产品)生理盐水溶液(1 mg/L),剂量为(15 μg/kg体重);ANP+NS组注射等容积生理盐水。(3)观察项目:术后3、6、12 h分别处死大鼠,取材观察下列指标:①肺湿/干重;②动脉血氧分压(PaO2,mm Hg);③肺泡灌洗液(bronchoalveolar lavage fluid,BALF)白细胞计数、蛋白质测定;④血浆内毒素含量,采用偶氮显色法测定;⑤血清TNFα含量,采用ELISA测定;⑥血清NO含量(μmol/L),采用硝酸还原酶法测定血清NO2-/NO3-的含量间接反映NO的生成量;
