【摘要】 目的外科临床护理论文 为肾脏闭合性损伤适合保守治疗的外科临床护理论文病例寻求全面、严密、有效的观察以及心理护理方案。从而早日使患者恢复健康。方法 对此类患者进行全面、严密的病情观察，从而及时了解病情的发展，避免病情延误，并通过积极良好的心理护理，改善患者心理状态，树立战胜疾病的信心。结果 本组病例，均及时了解病情进展，无病情延误发生，心理状态得到很好改善，从而早日恢复健康。结论 肾脏闭合性损伤适合保守治疗的病例，经过全面、严密、有效的病情观察以及心理护理，效果显著。

【abstract】 objective this thesis is suitable for guarding this kind of patient suffering from the closing injury of kidney in search of narrow,overall,and effective mental attending scheme,so that the patient will get well as soon as possible.methods to understand the development of patient′s condition in time,and avoid the delay of the condition,we watch this kind of patient carefully and roundly.and through the positive and favorable mental nursing,we improve the patient′s psychological condition,and establish the confidence of overcoming the disease,thus make the patient get well as soon as possible.results toward this group of patients,we all understand the development of patient′s condition in time,and don′t delay the patients.they all improve greatly in mentation.conclusion through narrow,overall,and effective mental attending scheme,this kind of patients suffering from the closing injury of kidney have improved greatly.

近年来随着工农业及交通业的蓬勃发展，造成肾脏损伤的机会显著增多。其中87%属闭合性损伤，大多经非手术治疗即可获得痊愈［1］。笔者对本病的观察及心理护理进行了总结、完善，为肾脏闭合性损伤适于保守治疗的病例寻求严密、全面、有效的观察及心理护理方案，从而使患者早日康复。
1 临床资料
1997年8月～2004年2月我院共收治闭合性肾损伤19例，年龄8～62岁，平均32.5岁；男17例，女2例，均为单侧发病。其中井下工伤11例，交通事故4例，坠落伤2例，硬物击伤2例。其中肾挫伤8例（伴肾包膜下血肿4例），肾部分性裂伤11例。合并其他脏器损伤情况如下：伴单侧肋骨骨折合并血气胸1例，锁骨骨折1例，腰椎压缩性骨折2例，脾破裂1例，跟骨骨折1例。其中伴有休克6例。本组病例均有明确外伤史，不同程度的肉眼血尿，少数仅为镜下血尿，伤侧肾区或上腹部疼痛。以及其他脏器损伤的相关临床表现。全组病例均经b超、肾脏ct、ivu、x线平片检查，从而明确诊断。肋骨骨折合并血气胸者，行胸腔闭式引流术+胸带外固定后行肾损伤保守治疗；锁骨骨折1例同时行双肩“8”字绷带固定；脾破裂1例，急诊行剖腹探查、脾切除术后转入我科；跟骨骨折1例，于保守治疗同时行患肢石膏外固定。
